We booked the Green Tour which included Derinkuyu. It is absolutely insane to think 20,000 people lived down here. The tunnels are tight, so be warned if you're claustrophobic, but it was the highlight of our trip. Our guide was essential to explain what the empty rooms were used for.
The history is incredible. The 'ticket only' option was good, but getting there by bus from Goreme is difficult, so I recommend a tour with transport. It is much deeper than Kaymakli. Wear good shoes because the stone floors are uneven.
The ventilation shaft is an architectural miracle. It's cool inside (13 degrees) so bring a sweater even in summer. We took the private tour option and it was worth it to move at our own pace without waiting for big groups in the narrow tunnels.
